Your role will be to work alongside social workers and other professionals to provide direct support to young people as identified to support their Care Plans and Pathway Plans. The role will involve a range of tasks including supporting young people to express their wishes and feelings, carrying out joint visits with social workers, supporting contact arrangements, completing Life Story Work and other tasks as required to support young people to successfully remain in their placements.
