The predicted salary is between 36000 - 60000 £ per year.

Contract: Fixed term until end of May 2026. Positive Steps is a unique organisation and has been changing the lives of people in Greater Manchester since 1977. Our current offer involves supporting those in Education, Criminal justice, Family and Community and Safeguarding settings. We are at an exciting point in our development with a focus on growing our impact while reaching excellence in all our work.

The main purpose of this role is to deliver holistic, bespoke packages of support to Children and Families in the borough of Oldham. Social Prescribers will work in a strength-focused way to undertake holistic assessments with children and young people and prepare joint person-centered Action Plans addressing a wide range of underlying issues as led by the requirements of the service. You will need to carry out specific interventions to support individuals to lead a positive lifestyle, to engage with community-based support and reduce their dependency and involvement with other services.

Social Prescribers will be aligned to one or more of the 5 cluster areas in Oldham and will work with colleagues and partners to develop a place-based offer for Oldham residents – working from a range of community venues including clients’ homes. For this role you will need to be confident in caseload management and the use of electronic recording systems.
