As a Commissioning Broker, you'll be at the heart of connecting people with the care they need. Acting as the link between families, professionals, and providers, you'll help secure high-quality care packages and placements that truly make a difference.
Responsibilities
- Support safe and timely hospital discharges, working closely with partners to ensure everything is in place
- Manage a diverse and sometimes complex caseload
- Maintain accurate and up-to-date records across systems
- Handle sensitive information with professionalism and confidentiality
- Monitor provider performance to maintain high standards of care
- Work collaboratively with managers to improve processes and outcomes
