The role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice. We are looking for 0.8 to 1 whole time equivalent enthusiastic and highly motivated clinical pharmacists to join our team and work with our existing Senior Clinical Pharmacist, Clinical Pharmacists, and Pharmacy Technicians. Completion of an independent prescribing course is an essential criterion for the role. Completion of the CPPE pathway is preferred but not essential, and there is an opportunity to support you through the CPPE pathway.

Citrus Health is a primary care network of 5 GP surgeries covering a patient population of 45,228 patients in the southwest of the Sandwell & West Birmingham CCG footprint. We are a dynamic and friendly team already experienced at working together to deliver excellent care to our patients. The successful candidate will be based at Portway Family Practice.
The successful candidate will have a higher degree in pharmacy and be a registered GPhC Independent Prescriber. They will also be expected to demonstrate completion of the national CPPE ARRS training pathway. The postholder will undertake face-to-face medication reviews for patients with polypharmacy, particularly older people, care home residents, and those with multiple co-morbidities. The role includes managing long-term conditions, working directly with patients to assess and treat conditions, and promoting self-care.
The successful applicant will work as part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, medicines reconciliation following transfer of care, and systems to support safer prescribing. The successful candidate will have excellent written and verbal communication skills, experience of influencing others, and expertise in conducting clinical medication reviews. They will also provide leadership in quality improvement and clinical audit while supporting delivery of the Quality and Outcomes Framework (QOF) and PCN Network Contract requirements. Experience working in General Practice or a Primary Care Network is preferred.
