The successful candidate will work in accordance with a rehabilitation philosophy that promotes maximum independence for all service users, supporting them to achieve their goals and regain confidence in daily living. They will welcome service users returning home from hospital, inpatient units or the community, completing initial assessments and ensuring a smooth transition with continuity of care. The role involves assessing and monitoring service users' progress, regularly reviewing and updating care plans in partnership with the multidisciplinary team and with the active involvement of the individual. The postholder will support service users with onward care arrangements, making referrals to Adult Social Care where appropriate and maximising the use of community resources to meet identified needs.
A key aspect of the role is providing practical support with a range of daily living activities, including mobility, transfers, personal care, household tasks, meal preparation, social engagement and access to community services and activities. The successful candidate will apply safe moving and handling practices at all times, ensuring the wellbeing and safety of both service users and colleagues. You will maintain accurate, timely and professional records, and supervise, support and monitor the performance of Reablement Support Assistants. In the absence of the Case Coordinator, the successful candidate will provide operational support and guidance to the team, escalating concerns and helping to ensure the smooth running of the service. They will work both independently and collaboratively, identifying and escalating risks appropriately to maintain high standards of care, safety and service delivery.
Responsibilities
- Perform initial assessments and facilitate smooth transitions for service users returning to the community.
- Regularly assess and monitor service users' progress and update care plans.
- Support service users with onward care arrangements and referrals to Adult Social Care.
- Provide practical support with mobility, transfers, personal care, household tasks, meal preparation, social engagement and community service access.
- Apply safe moving and handling practices at all times.
- Maintain accurate, timely, professional records.
- Supervise, support and monitor Reablement Support Assistants.
- Provide operational support and guidance to the team in the absence of the Case Coordinator.
- Identify and raise risks, ensuring high standards of care, safety and service delivery.
Qualifications
- Experience working with adults with care and support needs.
- Knowledge of relevant statutory framework, including the Care Act 2014.
- Experience delivering personal care.
- Ability to record, report, and raise information while maintaining confidentiality.
- Valid driving licence and access to a vehicle for community visits.
- Excellent communication skills and the ability to work flexibly to adjust to service needs.
- Right to work in the UK; no sponsorship will be offered.
- Disclosure and Barring Service (DBS) check required; exempt from the Rehabilitation of Offenders Act 1974.
Working Pattern
You are required to work on a rota basis covering Monday to Sunday as per the needs of the service, based in the office and completing community visits.
