We are seeking an experienced Social Worker who has completed the ASYE to assess the needs of adults with problematic substance use and plan outcomes under the Care Act 2014.
You will work in a multi-disciplinary team, supervise staff, and chair multi-agency risk management meetings, coordinating with health and social care services to promote independence and recovery.
