Senior Social Worker - Adults Intermediate Care Team

Salary: Band 10, Β£44.252- Β£48.738 - per annum

Location: Civic office although you will be required to work in other locations across the City such as QA Hospital, Summerlee and Spinnaker.

Your new role

The post is based within the Portsmouth Adults Intermediate Care Team (AICT) supporting our integrated services including Community Rehab Service, The Transfer of Care Hub, The acute hospital and Inpatient units.

Joining a team of Social Workers and Social Care Associate Practitioners you will be assessing the needs of adults and their Carers, to identify creative ways to meet their assessed outcomes under the Care Act 2014.

Using astrength-based approach you will support people to live the lives they chose. You will work collaboratively with others, maintaining a strength based and aspirational approach to achieve the outcomes the person wants.

Duties include assessing clients' needs and agreeing a person-centred support plan, completing reviews of assessments and support plans, supporting people with their health, mental health, and social care needs, and assessing mental capacity when required. You will work independently, manage your own caseload, and have opportunities to support unregistered/registered staff and pursue your own personal learning and development. The safeguarding of adults is an important and integral area of your work as will be the implementation of the safeguarding policy and procedures.

Who is the person summary:

You will be qualified and registered with Social Work England with significant experience of working with adults in a statutory social work fieldwork setting, including substantial involvement in Safeguarding Adults. With a thorough understanding and working knowledge of legislation governing adult social care, you will have demonstrable experience of managing complex cases, undertaking risk assessment and decision-making, and providing professional support, guidance, and oversight to less experienced practitioners. With strong communication skills, you will have the ability to advocate for individuals and carers as well as experience of supervising and supporting staff which is essential.
