The purpose of this post is to support children in care and care leavers to achieve the best possible outcomes as they transition into adulthood. You will work as part of a dedicated team to implement care and pathway plans, ensuring young people are prepared for independent living.
Key Responsibilities
- Direct Support: Provide practical and emotional support to children, young people, and their carers.
- Pathway Planning: Act as a case manager for care leavers up to age 21 (or older if in education), helping to draft and review their statutory pathway plans.
- Advice & Guidance: Offer informed, unbiased advice on education, training, employment, and accommodation.
- Partnership Working: Liaise with Health Authorities, Adult Social Care, and Housing providers to ensure young people receive their full entitlements.
- Participation: Support the Children in Care Council and Care Leavers Forum to promote the active participation of young people.
