The purpose of this post is to support children in care and care leavers to achieve the best possible outcomes as they transition into adulthood. You will work as part of a dedicated team to implement care and pathway plans, ensuring young people are prepared for independent living.

Key Responsibilities:

  • Direct Support: Provide practical and emotional support to children, young people, and their carers.
  • Pathway Planning: Act as a case manager for care leavers up to age 21 (or older if in education), helping to draft and review their statutory pathway plans.
  • Advice & Guidance: Offer informed, unbiased advice on education, training, employment, and accommodation.
  • Partnership Working: Liaise with Health Authorities, Adult Social Care, and Housing providers to ensure young people receive their full entitlements.
  • Participation: Support the Children in Care Council and Care Leavers Forum to promote the active participation of young people.

About You (Person Specification)

Essential Criteria:

  • Qualifications: NVQ Level 3 in Childcare or an equivalent qualification.
  • Experience: Significant experience working professionally with children, adolescents, or families.
  • Skills: Strong verbal and written communication skills, with the ability to build purposeful relationships and manage conflicting interests or distress.
  • Knowledge: A solid understanding of child development and the legislative framework surrounding looked-after children.

Requirements:

  • A satisfactory enhanced DBS disclosure.
  • A willingness to work outside of normal office hours when the service requires.
  • A commitment to safeguarding, equality, and anti-oppressive practice.

General Requirements

You will be expected to maintain accurate case records, participate in regular supervision, and contribute to the ongoing development of the service to improve outcomes for our young people.
