We are seeking an experienced and highly motivated Advanced Clinical Practitioner (ACP) to join our friendly and forward-thinking GP practice team. This is an exciting opportunity for a clinician who is passionate about delivering high-quality, patient-centred care within primary care. The successful candidate will work autonomously and as part of a multidisciplinary team, providing advanced clinical assessment, diagnosis, and treatment of patients presenting with a wide range of conditions.
Main duties of the job
- Provide expert clinical care, including history taking, physical examination, diagnosis, and management.
- Independently assess, diagnose, and manage acute and chronic conditions.
- Prescribe medications safely and effectively as an independent prescriber.
- Manage undifferentiated and complex patient presentations.
- Deliver health promotion and disease prevention advice.
- Work collaboratively with GPs, nurses, pharmacists, and other healthcare professionals.
- Contribute to service development and quality improvement initiatives.
- Maintain accurate and contemporaneous patient records.
Qualifications
- Registered healthcare professional (e.g. NMC, HCPC).
- MSc in Advanced Clinical Practice (or working towards is optional).
- Significant post-registration clinical experience.
- Experience working in primary care or urgent care settings.
- Advanced clinical assessment and decision-making skills.
- Ability to work autonomously and manage clinical risk.
- Experience in GP practice settings.
- Non-medical prescribing experience within primary care.
- Interest in long-term condition management.

Job responsibilities
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN or practice team.
- Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services and to self-care.
- Be able to perform specialist health checks and reviews.
- Perform and interpret ECGs; alongside other results as appropriate.
- Perform investigatory procedures as required.
- Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.
- Perform investigatory procedures needed by patients and those requested by GPs and the wider PCN.
- Perform a domiciliary (home) visit.
- Support the delivery of anticipatory care and end of life care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).
- Provide an alternative model to urgent and same day home visits for the PCN and undertake clinical audits.
- Communicate at all levels across PCNs and other organisations, ensuring effective, patient-centered service.
- Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services where appropriate.
- Prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice N.B relevant if possesses prescribing qualification only.
- Assess, diagnose, plan, implement and evaluate treatment and or interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs within the clinical and domiciliary setting.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Take an active part in achievement of contractual markers.
- Implement and participate in vaccination programmes as appropriate.
Person Specification
- Advanced Clinical Practitioner able to operate at an advanced level of clinical practice.
- Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11.
