POPS is working in partnership with Liverpool City Council and HMP Liverpool to ensure that prisoners and their families who reside in the Liverpool area have supported access to services in the community to help with matters relating to their family and children, e.g., practical and emotional support, relatives caring for your children, matters such as housing, money worries, school attendance, and parenting support.

Based within the prison, the purpose of the role is to promote the community based services available across Liverpool, to engage with prisoners and their family members to identify support needs, to liaise with and work in collaboration with Liverpool City Council Link Workers and to make supported referrals into the existing Local Authority services and Family Hubs in Liverpool.

MAIN TASKS

  • To work directly inside HMP Liverpool.
  • Ensure all prisoners at HMP Liverpool who have a family with an ‘L’ postcode are made aware of the support they and their families can access at Liverpool Family Hubs and from Liverpool City Council Link Workers.
  • Inform and support prison mentors and prison key workers to understand the project and how to make referrals.
  • Work alongside and in collaboration with Liverpool City Council’s Link Workers (who are based in the prison).
  • Inform and support prisoners and families with an ‘L’ postcode to understand how to access Liverpool City Council community based support offer.
  • Receive referrals from prisoners requesting one to one support relating to their children and families support needs (Liverpool based families).
  • Engage in 1-1 meetings with prisoners to identify their children and family support needs.
  • Engage directly with eligible family members either over the phone or in the prison visit centre where appropriate.
  • Obtain consent to contact their nominated family member who may require support.
  • Obtain consent from nominated family member and/or prisoner to share information and contact details with appropriate community based services.
  • Ensure due process is observed: Public Protection and Offender Management Unit prior to any referrals to external agencies being referred.
  • Actively support and encourage prisoners and/or their family members to engage with trained link workers (Liverpool City Council Link Workers).
  • Ensure all paperwork is accurately completed.
  • Maintain accurate case management information using POPS’ IT based CRM system (Charity Log).
  • Monitor progress of referrals and maintain contemporaneous records on POPS in house CRM system (Charity Log).
  • Complete the Liverpool Family Hub Monthly Monitoring report and link in with POPS’ Lead Family Engagement Worker to ensure that data is shared and feeds into the monthly report.
  • Ensure prison based agencies such as Offender Management Unit, Health Care and other departments understand the project and how to make referrals.
  • Attend and contribute to prison based and other external meetings as and when required.
  • Participate in training that is considered relevant to the post.
  • Cover other areas of POPS’ service delivery as required.
  • Ensure all POPS’ and prison policies and procedures are adhered to.
  • Carry out additional relevant tasks as assigned by line manager.
