Location: London

LCS is seeking a committed and enthusiastic Outreach Support Worker to work on a casual basis, covering shifts within the business as and when needed. This opportunity offers flexibility to work the hours you can commit to, with no contractual obligation. As an Outreach Support Worker, your role involves providing a responsive, customised, and quality service to young people placed in their individual flats or shared houses. You will be travelling around Watford, Harrow, and Hatfield, supporting young people within the community and in their supported living homes. This could include attending daily appointments such as the Home Office, Doctors, Shopping, and supporting their journey towards independent living.

Our vision is to make a positive and lasting difference to young people who have been in care and are moving towards independence. We provide accommodation and support to help young people achieve independence and educational success.

Young people supported by Leaving Care Solutions are vulnerable, and you are responsible for ensuring that the environment you work in provides a safe, welcoming, and healthy space for them.

Duties will include:

  • Working in line with person-centred risk assessments and support plans, liaising with support workers for full handovers.
  • Observing changes in behaviour, documenting appropriately, and reporting support needs.
  • Maintaining positive relationships with young people, staff, and professionals through effective communication and teamwork.
  • Providing continuous support to young people, focusing on safety and well-being.
  • Carrying out health and safety tasks, including cleaning, following policies and procedures.
  • Liaising with external agencies such as police and Emergency Duty Team (EDT).
  • Supporting young people attending hospital or police stations.

All applicants must have:

  • Experience supporting vulnerable young people.
  • Flexibility to work weekends.
  • Qualifications such as Residential Childcare/Families Practitioner Diploma/Children and Young People\’s Workforce/NVQ Level 3 in Health & Social Care or equivalent, or willingness to study towards this.
  • A full driving license and access to a vehicle, as travel is required.
