POSITION SUMMARY: The Licensed Social Worker must promote positive morale throughout the facility. The role involves working with residents, families, and team members to outline and meet care goals, and facilitate discharge planning.

RESPONSIBILITIES:

• Ensure resident adjustment to care plans through screenings, initial care planning, and paperwork including MDS.
• Review Resident Rights with new admissions.
• Complete admission agreements.
• Assist residents with applications for financial assistance, collaborating with families, the Business Office, and Medicare/Medicaid.
• Schedule and lead interdisciplinary team meetings to plan and coordinate care.
• Provide guidance and counseling to residents and families on issues like grief, loss, and coping strategies.
• Coordinate discharge services and follow-up care.

QUALIFICATIONS:

• 1-3 years of long-term care experience.
• Bachelor\’s Degree in Social Work, Psychology, or a related field.
• Social Work Licensure (LSW, LCSW, or LICSW).
