The Vine Day Service provides a variety of centre and community‑based opportunities on a daily basis (Monday – Friday) for up to 50 adults with learning disabilities and a range of complex health and social care support needs. A modernised facility called Meadow View will soon support day services and respite located near central park.
The Community Outreach Team provides time‑limited support to people identified across the city who need hands‑on advice and guidance relating to:
- Housing
- Managing debts, budgets and finances
- Support with form filling
- Support to access the community
- Independent travel
- Introduction to a range of local community‑based activities
- Support for introduction to specific organisations that can assist the person longer term
Develop and implement your own working day plans, drawing up strategies to best support the person you are allocated to.
