Responsibilities
- The role of Community Outreach Worker is to support the care management process, working to support adults with a variety of needs in order to promote independence, health and wellbeing.
- The role includes supporting people to improve their quality of life, whilst encouraging the person to develop wider links within the community.
- The post holder will be responsible for signposting and facilitating involvement with a range of services and agencies via research, advice, information and support in the local/wider community.
Qualifications
- Knowledge and experience of the care planning and review process.
- The post holder will have knowledge of Care Management and the principles of Community Care and other relevant legislation.
- NVQ3 in Care / Promoting Independence, or equivalent qualification.
