Your Impact:

In this vital role, you'll provide person-centered support, fostering independence and enhancing the well-being of individuals within a residential supported living setting. Your contributions will directly impact their quality of life, enabling them to thrive within their community.

Key Responsibilities:

  • Holistic Support: Deliver individualised care plans that address the unique needs of each service user, encompassing physical, emotional, and social well-being.
  • Daily Living Assistance: Provide sensitive support with personal care, medication administration, meal preparation, and household tasks, promoting dignity and autonomy.
  • Community Integration: Facilitate meaningful engagement in community activities, appointments, and social events, fostering inclusion and reducing isolation.
  • Mental Health Support: Provide empathetic support to individuals experiencing mental health challenges, working alongside professionals to promote emotional well-being.
  • Accurate Documentation: Maintain detailed and accurate records, including daily logs, incident reports, and support plan updates.
  • Collaborative Teamwork: Work effectively as part of a multidisciplinary team, collaborating with families, healthcare professionals, and other stakeholders.
  • Flexible Scheduling: Adapt to a flexible work schedule, including weekends, evenings, and sleep-in duties.

Essential Requirements:

  • Proven experience supporting adults with learning disabilities, epilepsy, and acquired brain injuries.
  • Demonstrated understanding and experience in supporting individuals with mental health needs.
  • Exceptional communication and interpersonal skills, fostering positive relationships with service users and colleagues.
  • Previous experience in care assistance, social care, or working with vulnerable adults.
  • Ability to work independently and as part of a collaborative team.
  • Commitment to providing high-quality, person-centered care.
  • Willingness to participate in ongoing training and professional development.
  • A NEW enhanced DBS check will be required.

Desirable Skills & Qualifications:

  • Relevant qualifications in health and social care.
  • MHFA (Mental Health First Aid) Certification.
  • Full UK driving license.
