In line with relevant training / qualifications held, deliver specific Healthy community project/programmes, which may include: Exercise Referral, Good Boost MSK Rehabilitation, Move Through Menopause, Cardiac Rehabilitation, Cancer Rehabilitation, Falls Prevention, and Big Sister. Lead classes designed for adults with health conditions and limited mobility, ensuring a safe, inclusive, and supportive environment. Support community outreach events to engage new participants and raise awareness of programmes. Book, coordinate, and manage appointments and programme sessions efficiently. Assist the team in onboarding participants and helping them access the full range of Healthy Communities programmes.
Responsibilities
- Complete any monitoring and evaluation requested by line manager in line with current data protection protocols, including case studies and questionnaires.

- Qualified instructor in relevant discipline to deliver sessions independently i.e. Level 3 GP Referral, Cancer Rehabilitation, Cardiac Rehabilitation, Postural Stability Instructor, Exercise for Neurological Conditions.
- Experienced in planning, delivering and monitoring sessions.
- Passion for improving the health of the local community.
- Ability to use data management systems.
