About the Role
Our client is seeking a motivated Community Health Navigator to join their remote team. In this critical role, you will empower individuals to access and understand healthcare services, addressing barriers and facilitating connections to care. Working entirely from home, you will utilize virtual communication channels to guide clients through the healthcare system, ensuring they receive the support they need to manage their health effectively. This position is essential for improving health outcomes and promoting wellness within the community, particularly for underserved populations in the Gauteng area, delivered remotely.
Key Responsibilities
- Conduct virtual health needs assessments and provide personalized guidance on accessing healthcare services.
- Assist clients in navigating insurance options, appointment scheduling, and understanding medical information.
- Connect individuals with appropriate healthcare providers, specialists, and community health resources.
- Provide education on preventive health measures and chronic disease management strategies.
- Maintain confidential electronic client records and track progress towards health goals.
- Collaborate with healthcare providers and social service agencies to coordinate client care.
Requirements
- Bachelor's degree in Public Health, Health Sciences, Social Work, or a related field.
- Minimum of 3 years of experience in health navigation, community health work, or a related role.
- Demonstrated experience in virtual client interaction and support.
- Strong knowledge of the South African healthcare system and community resources.
- Excellent communication, empathy, and problem-solving skills.
- Proficiency with telehealth platforms and digital record-keeping systems.
