About the Complex Care Mobilisation Coordinator Role: As a Complex Care Mobilisation Coordinator, you'll work closely with our Clinical, Recruitment, and Coordination teams to mobilise new care packages efficiently and effectively. This involves scheduling assessments, assigning care staff, preparing rotas, and maintaining compliance records. You'll be the key point of contact for clients, families, and commissioners, providing updates and escalating issues as needed.

Key Responsibilities:

  • Coordinate new complex care package mobilisation under senior guidance
  • Communicate effectively with clients, families, commissioners, and internal teams
  • Schedule client assessments in partnership with the Clinical Team
  • Work with Recruitment and Coordination to assign care staff
  • Create staff profiles, arrange introductions, and prepare initial rotas
  • Maintain mobilisation records in care management systems
  • Ensure compliance with CQC guidelines and company policies
  • Provide updates to stakeholders, escalating issues as needed
  • Hand over care packages smoothly to the long-term care team

Skills and Experience Required:

  • Experience in care coordination, administration, or mobilisation
  • Knowledge of complex care and CQC standards
  • Strong communication and relationship-building skills
  • Excellent time management and attention to detail
  • Proficiency in Microsoft Office and rostering systems
  • NVQ Level 2+ in Health & Social Care (or equivalent)
