Key Responsibilities- Evaluate patients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
- Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
- Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
- Communicate regularly with physicians, addressing any changes in patient condition.
- Participate in the periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required.
- Educate patients and families/caregivers on speech therapy techniques and related challenges.
- Confirm daily visit schedules to ensure seamless coordination with other staff.
- Document all evaluation data, treatments, and patient progress promptly.
- Supervise home health aides every 14 days when speech therapy is the sole discipline involved.
- Must have a current SLP license (state-specific).
- Graduate of an accredited speech language pathology program.
- Valid BLS certification.
- Minimum two years of speech therapy experience (home health experience preferred).
- Excellent communication, organizational, and time management skills.
- Must have a valid driver’s license and access to a reliable and insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
