The Opportunity: Pine Park Health is hiring Nurse Practitioners (Full-Time) to revolutionize primary care for seniors in the East Bay area. What You'll Do:
- Provide comprehensive onsite primary care to seniors in assisted living and senior living communities.
- Develop and execute personalized, patient-centered care plans that address the whole person.
- Partner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolves.
- Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation.
- Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes.

The base salary range for this role is $135,000 to $160,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
