We are seeking a dedicated Assessment & Reconnection Worker to support individuals who are homeless, at risk of homelessness, or new to rough sleeping. You will carry out assessments, develop personalised support plans, and help clients reconnect with accommodation, support networks and services to achieve positive long-term outcomes.
Key Responsibilities
- Conduct comprehensive assessments and develop tailored action plans with clients.
- Provide ongoing case management, support and advocacy.
- Support clients to access accommodation, benefits, healthcare and other essential services.
- Coordinate reconnection arrangements, including travel, accommodation and support packages.
- Work closely with local authorities, outreach teams and partner agencies.
- Assess risks and develop appropriate risk management plans.
- Maintain accurate case records and update electronic databases.
- Monitor outcomes and identify barriers to successful reconnection and move-on.
About You
- Experience working directly with the public, customers or vulnerable individuals in a busy environment.
- Understanding of homelessness, housing support or the challenges faced by vulnerable people.
- Ability to manage complex behaviours and support individuals in crisis.
