Our client is a large NHS Healthcare Provider of Community based Healthcare services to more than 1.2 million people living in Hertfordshire and surrounding areas.
Position Overview: – To Provide community healthcare services to patients living in the Hertfordshire area. We are seeking an experienced and compassionate Community Nurse to provide high-quality nursing care to patients within their own homes or community settings. The role involves working closely with multidisciplinary teams to deliver personalised care, promote independence, and support patients with acute, chronic, or palliative needs.
Key Responsibilities:
* Assess, plan, implement, and evaluate individualised care plans.
* Administer medications, wound care, injections, and other nursing procedures.
* Monitor patients’ conditions and escalate concerns appropriately.
* Support hospital discharge processes and prevent unnecessary admissions.
* Educate patients, families, and carers on self-care and health management.
* Maintain accurate clinical documentation in line with NMC standards.
* Work collaboratively with GPs, social services, and allied health professionals.
What We’re Looking For:
* Registered Nurse (Adult) with the Nursing and Midwifery Council (NMC).
* Previous experience in community or district nursing is desirable.
