Tice Valley Post Acute is seeking an experienced, compassionate, and highly organized Social Services Director to join our dedicated interdisciplinary team. This key leadership role serves as a vital advocate for our residents, helping ensure their emotional, psychological, and social needs are met with dignity, empathy, and professionalism. The ideal candidate is calm under pressure, maintains the highest standards of confidentiality, and demonstrates exceptional emotional intelligence when supporting residents and families through complex care decisions and life transitions.

Key Responsibilities

  • Resident Advocacy & Care Planning: Assess residents’ psychosocial and emotional needs and develop individualized care plans. Support residents in achieving maximum independence and quality of life.
  • Emotional & Family Support: Provide counseling, guidance, and emotional support to residents and families coping with illness, disability, grief, and end-of-life challenges.
  • Clinical & Ancillary Coordination: Collaborate with the interdisciplinary care team regarding resident healthcare decisions. Coordinate ancillary services including psychiatry, dentistry, podiatry, and optometry.
  • Discharge Planning & Community Resources: Lead discharge planning efforts to ensure safe and successful transitions. Coordinate referrals, equipment needs, and community-based support services.
  • Case Management: Assist residents in securing prosthetics, assistive devices, and additional community resources as needed.
  • Administrative & Regulatory Support: Maintain accurate resident documentation and reports. Investigate and manage concerns related to missing belongings. Provide staff education and in-service training when appropriate.
  • Collaboration & Communication: Foster strong partnerships with residents, families, healthcare providers, and external agencies to ensure seamless continuity of care.

Qualifications

  • Excellent communication, interpersonal, and active listening skills.
  • Ability to manage sensitive situations with professionalism, discretion, and objectivity.
  • Strong organizational, planning, and problem-solving abilities.
