Position Summary

As an MDS Nurse (LPN), you will be responsible for coordinating and completing Minimum Data Set (MDS) assessments in compliance with federal and state regulations. This role directly impacts care planning, regulatory compliance, and Medicare/Medicaid reimbursement.

Qualifications

  • Active, unencumbered RN license in South Carolina (required)
  • Experience in a skilled nursing or long-term care setting preferred
  • Strong knowledge of MDS 3.0, RAI process, and reimbursement guidelines
  • Experience with EHR systems (PointClickCare preferred)
  • Proficiency in Microsoft Office (Excel, Word, Outlook)
  • Strong attention to detail, organization, and analytical skills
  • Ability to collaborate with interdisciplinary teams
