Your Impact & Responsibilities
- Bedside Excellence: Provide high-quality nursing care, including medication administration, wound treatments, and resident assessments.
- Clinical Monitoring: Closely monitor resident vitals and conditions, promptly reporting any changes to the RN or physician.
- Care Coordination: Collaborate with our in-house therapy team (PT/OT/SLP) and physicians to ensure comprehensive recovery plans.
- Documentation: Maintain precise, timely, and compliant electronic medical records (PointClickCare experience is highly valued).
- Leadership: Provide direction and mentorship to CNAs to ensure resident ADLs are met with the highest standards of care.
- Advocacy: Act as a compassionate resource for residents and their families during their transitional care journey.
Qualifications
- License: Must possess a current, valid California Licensed Vocational Nurse (LVN) license.
- Certification: Current CPR/BLS Certification.
- Experience: Previous experience in subacute, skilled nursing, or transitional care is preferred.
- Skillset: Strong clinical judgment, excellent communication skills, and a 'team-first' attitude.
