Key Responsibilities
- Clinical Care
- Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs
- Deliver compassionate, resident‑centered care focused on comfort, dignity, and safety
- Assessment & Monitoring
- Observe, assess, and document changes in resident condition
- Communicate concerns promptly to the RN Supervisor and healthcare providers
- Care Planning
- Assist with the development and implementation of individualized care plans
- Support resident recovery goals and quality‑of‑life initiatives
- Leadership & Team Support
- Supervise and support Certified Nursing Assistants (CNAs)
- Collaborate with interdisciplinary team members to ensure continuity of care
- Documentation & Compliance
- Maintain accurate clinical documentation and medical records
- Ensure compliance with all state, federal, and facility regulations and standards
Qualifications
- Current and active California LVN license required
- Current BLS/CPR certification required
- Previous skilled nursing or post‑acute care experience preferred
- Strong clinical judgment and communication skills
- Compassionate, dependable, and team‑oriented approach to resident care
