Key Responsibilities

  • Clinical Care
    • Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs
    • Deliver compassionate, resident‑centered care focused on comfort, dignity, and safety
  • Assessment & Monitoring
    • Observe, assess, and document changes in resident condition
    • Communicate concerns promptly to the RN Supervisor and healthcare providers
  • Care Planning
    • Assist with the development and implementation of individualized care plans
    • Support resident recovery goals and quality‑of‑life initiatives
  • Leadership & Team Support
    • Supervise and support Certified Nursing Assistants (CNAs)
    • Collaborate with interdisciplinary team members to ensure continuity of care
  • Documentation & Compliance
    • Maintain accurate clinical documentation and medical records
    • Ensure compliance with all state, federal, and facility regulations and standards

Qualifications

  • Current and active California LVN license required
  • Current BLS/CPR certification required
  • Previous skilled nursing or post‑acute care experience preferred
  • Strong clinical judgment and communication skills
