Are you passionate about improving care for adults with neurodevelopmental conditions? Excited by the opportunity to shape a brand-new service and make a genuine difference from day one? If so, we'd love you to join our growing Primary Care Network team! We are establishing an innovative, forward-thinking neurodevelopmental service within primary care, and are looking for an enthusiastic and motivated Neurodevelopmental Practitioner to help us bring it to life. You will be joining a friendly and highly supportive team that currently includes two GPs, two Advanced Practitioners, and a care coordinator, with big plans to expand and develop the service over the coming years.
In this varied and rewarding role, you'll be the first point of contact for adults seeking support around ADHD and associated challenges. You'll use your skills to complete needs-led assessments, provide evidence-based guidance and education, and help people navigate their options with confidence. Under clinical supervision (and with full training provided), you'll also play a key role in delivering ongoing ADHD reviews, including physical health checks and monitoring of symptoms and treatment needs. This post is full-time; however, part-time applications will be considered as part of a job-share arrangement for the right candidates.
Main Duties of the Job
- Receive, triage and manage referrals for adults requiring ADHD assessment, review and ongoing treatment.
- Undertake comprehensive ADHD assessments and annual reviews in line with service protocols and best practice guidance.
- Provide evidence-based education, advice and support to help patients understand and manage ADHD symptoms.
- Manage a caseload of patients, working collaboratively with GPs, primary care colleagues, carers and external agencies.
- Maintain accurate and timely clinical records, making recommendations for ongoing care and onward referrals where appropriate.
- Contribute to the delivery of a holistic, patient-centred service that considers both physical and mental health needs.
- Support integrated working across primary care, specialist services and the wider multidisciplinary team.
About us
Picton PCN consists of 6 GP practices that cover 50,000 patients. We have a large Network team that consists of Pharmacists, Physios, Paramedics, Social prescribers, Care Coordinators, and a Mental Health team. We work together to provide quality care by delivering clinical appointments in our GP practices and health events in the community to our patients.
Job responsibilities
Patient Assessment & Clinical Care
- Receive, screen, stratify, and manage referrals for adults requiring ADHD assessments, review or ongoing treatment.
- Conduct face-to-face, and where appropriate, remote consultations.
- Undertake comprehensive needs-led assessments in line with current guidance, best practice, and service protocol.
- Provide evidence-based education, advice, and guidance to support patients in understanding and managing ADHD symptoms.
- Undertake ADHD annual reviews (under appropriate supervision), including physical health monitoring, symptom review, medication side-effect discussions, and support with non-pharmacological strategies.
- Maintain a holistic, person-centred approach that considers both physical and mental health needs, signposting or referring to other services where appropriate.
- Coordinate a range of investigations or monitoring required to support patient care.
Caseload Management
- Manage a defined caseload of patients, working collaboratively with healthcare professionals, carers, and external agencies to ensure appropriate support is in place.
- Develop an understanding of the patient journey from referral through assessment, treatment, and ongoing management within primary care and shared care agreements.
- Support patients to access appropriate services and resources throughout their care pathway.
- Build effective working relationships with GPs, practice teams, secondary care providers, and wider multidisciplinary teams.
- Liaise with primary care, social care, and specialist services to ensure coordinated patient care.
- Support integrated working across practices, PCN services, and secondary care through effective communication and partnership working.

Communication & Collaborative Working
- Build effective working relationships with GPs, practice teams, secondary care providers, and wider multidisciplinary teams.

Clinical Records & Quality Assurance
- Maintain clear, accurate, and timely clinical records, documenting assessments, interventions, advice provided and recommendations for ongoing care.
- Ensure all work is delivered in accordance with relevant clinical guidance, governance requirements, and PCN policies.
- Contribute to maintaining and improving the quality of service provision.
Professional Development & Continuous Improvement
- Keep up to date with developments in ADHD assessment, treatment, mental health, and relevant research.
- Engage with online resources and maintain knowledge materials that support both colleagues and patients.
- Participate in supervision, appraisal, and continuous professional development activities.
- Complete all mandatory and statutory training requirements.
- Work autonomously within agreed clinical supervision arrangements, and the ADHD service model.
- Assist in the preparation of regular reports for internal and external stakeholders, including the PCN and ICB.

Knowledge
- Evidence of extensive professional development relevant to the role.
- Ability to work independently with defined policies and procedures, with clinical supervision from the line manager.
- Ability to engage with people with neurodiversity effectively, assessing their needs, assessing and understanding risk, and designing and delivering appropriate interventions, evaluating their impact to inform future interventions and their personal practice.
- Developed communication and coaching skills to aid understanding, motivating others, or negotiating agreement.
- An extensive knowledge and understanding of neurodevelopmental conditions.
- Knowledge of common mental health conditions.
- Confidence and credibility to hold challenging discussions with patients.
- Ability to write clear appointment notes, data entry, and letters to other clinical settings.
- High level communication skills, including an ability to communicate and work in settings in which the atmosphere may be emotive.
- Problem solving skills and ability to respond to sudden unexpected demands.
- Ability to work collaboratively as part of a team.

- Experience of working in or with or alongside a PCN or GP Practice.
- Knowledge of ARRS roles, personalised care, and population health management.
- Knowledge of current issues in the NHS and wider health and social care sector.
- Experience of using GP clinical systems, such as EMIS.
Qualifications
- A degree in Psychology, Health, Social Care or a related field, or demonstrate equivalent experience through formal qualifications (minimum Level 3) and substantial experience in neurodevelopmental conditions, mental health, or primary care settings.
- Professional registration (e.g. NMC, HCPC, BACP).
