Phyllis Tuckwell (PT) is seeking a compassionate and motivated Team Leader to support the delivery of high-quality palliative and end of life care within our communities. As a Team Leader within the PT multidisciplinary Hospice at Home team, you will be responsible for providing clinical leadership and monitoring the delivery of high quality and effective nursing care to patients in the community who require palliative and end of life nursing care.

Working alongside other Team Leads you will provide accessible, and visible leadership and effective day-to-day operational line management, including a planned night service staffed by HCAs, over 7 days a week. You will be required to work as an autonomous practitioner and display professional curiosity, being able to make clinical decisions. The role will take an active part in the Co-ordination function, clinically prioritising referrals and allocating resources according to clinical need. Our Team Leads have an allocated locality which mirrors other external community services, and are responsible for, and lead on, areas of practice including quality improvement, risk, and people management. Effective team working skills are essential, to ensure continuous improvement and good information sharing.

Responsibilities

  • Be a Registered Nurse
  • Possess strong leadership and people management skills, being able to supervise, mentor and develop team members
  • Display excellent communication and psychological skills
  • Have the ability to work autonomously
  • Have demonstrable clinical skills, knowledge, and experience in Palliative Care
  • Hold the ability to lead on day to day operational & staff management skills in clinical & information governance and risk management
  • Possess quality improvement skills with the ability to support the review of guidelines and procedures
  • Have proven skills in undertaking audit and service development projects

We are based in Farnham, Camberley and Guildford, and provide bespoke, compassionate palliative and end of life care for people living with an advanced or terminal illness, across West Surrey and North-East Hampshire.
