£36,060-£40,076 WTE based on 37.5 hours a week. 38 hours per week in Farnham & Guildford and across our catchment area.

We are looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. You will deliver the highest standards of palliative and end‑of‑life nursing care to patients, their families and carers in the community, providing holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with the Phyllis Tuckwell multidisciplinary team and colleagues across health and social care. This role requires a strong commitment to enabling choice and providing respectful, dignified support for people as they approach the end of life and for those close to them. You will contribute to a safe, effective and efficient culture by participating in risk assessment and working in line with PT policies and procedures, helping to ensure a seamless experience for every patient. Job responsibilities

Clinical

  • Assess, plan, coordinate, implement and evaluate palliative care interventions based on an agreed plan of care that supports quality and timely care delivered to the patient and family or carer.
  • Ensure that care is centred around the person, family and their carer and that effectiveness, clinical safety and a joint approach with community nursing colleagues underpins PT practice.
  • Work closely with PT colleagues, the District Nursing teams and other partners in the provision of personalised holistic care to patients, their families, and carers.
  • Ensure quality standards are maintained and constantly re‑evaluated, to provide the highest standards of holistic care.
  • Ensure that clear and concise reports of clinical and other activity are maintained and monitor record keeping standards in accordance with NMC guidelines, community and PT policy.
  • Be able to demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace.
  • Act as a resource and support to palliative care colleagues through giving advice, information, support and education.
  • Record clinical activity as required by the organisation and practice with due regard to confidentiality and data protection.
  • Report risks, accidents, incidents, negative feedback and complaints to the Team Lead/Manager the same day.
  • Work with colleagues to assess their clinical competencies as appropriate, providing training and support.

Professional

  • Work in accordance with NMC Code and within the NMC standards.
  • Contribute to team cohesion, through ongoing supportive relationships and consideration of colleagues.
  • Make autonomous decisions that prioritise the patient's best interests whilst practising safely within professional and personal limitations.
  • Identify and work to mitigate risks, and report accidents or incidents in a timely way, ensuring appropriate action and the optimum safety and wellbeing of patients, their families and colleagues.
  • Develop and maintain effective relationships with members of the Primary Health Care Team and all other agencies involved in delivering care in the community.
  • Understand and work within relevant legislation and PT philosophy, mission and values.
  • Liase with professionalism and respect towards all members of the multidisciplinary team who contribute towards the Hospice at Home Team and the wider team supporting the patient and their family at home.
  • Represent Phyllis Tuckwell in the public arena and occasionally give educational talks ensuring that the reputation of PT is maintained and enhanced.

Managerial

  • Be familiar with guidelines and regulations relevant to the role and case‑setting, particularly those relating to clinical and information governance, health and safety at work, and ensure they are met.
  • Adhere to all Phyllis Tuckwell policies and procedures and promote this with the wider team.
  • Be aware of the security of the Phyllis Tuckwell (PT) buildings in and out of office hours and report any concerns to the on‑call maintenance team or on‑call manager.
  • Contribute towards ensuring that exposure to risk is minimised through safe practice and awareness of health and safety and infection prevention and control.
  • Report any feedback, concerns and complaints from patients, their relatives, or others acting on their behalf, to the line manager.
  • Report any incidents or accidents, managing these via the reporting system and escalating them as appropriate.
  • Ensure communication between members of the Hospice at Home team is effective so the care needs to patients are easily transferred to the person or persons assuming continuity of care at shift changeovers.

Training and Development

  • Support a team approach through being proactive and positive.
  • Act as an assessor and mentor to students who need to develop skills and acquire experience in the field of palliative care.
  • Support the professional development and safe practice of others.
  • Identify, develop and maintain own knowledge and skills through continual professional development, discussion with an assessor, and through PT annual appraisal process.
  • Maintain up‑to‑date clinical experience and knowledge and be able to provide evidence of this for nursing revalidation.
  • Strengthen and develop clinical skills across all care settings in conjunction with line manager.

Research

  • Participate in research and audit within PT where appropriate.
  • Implement findings of evidence‑based practice.

Safeguarding, Equality, Diversity and Inclusion

  • Adhere to Phyllis Tuckwell's Safeguarding Policy (held on the NEST), which supports the local authorities' multi‑agency safeguarding policy, the law (Care Act 2014) and Mental Capacity Act 2005 (including Deprivation of Liberty Safeguards).
  • Inform the appropriate Phyllis Tuckwell professional where there is concern that an adult or child may be at risk of harm, abuse, or neglect.
