Consultant in Palliative Medicine in Farnham

Hours: 24 - 40 hours per week (part‐time applicants welcome to apply)

Salary Range: £109,725 - £145,478 per annum

Base: The Inpatient unit, Farnham (occasionally at Beacon Centre, Guildford)

Accountable to: Medical Director

Phyllis Tuckwell is looking to appoint a Consultant in Palliative Medicine to work as part of an established team of consultants in a modern technology‐enabled teaching hospice. The post comprises a minimum of 6 and a maximum of 10 programmed activities (PAs). Direct clinical care (DCC) will be delivered predominantly on the Inpatient unit initially alongside an experienced consultant. In time, the role will broaden to community‐based work as part of the Hospice at Home (H@H) team, depending on experience and confidence. The role also includes supporting professional activities (SPA) time dedicated to service development and delivery of educational activities to the organisation's multidisciplinary team and external colleagues.

Main Duties & Responsibilities

• Clinical
  • Provide consultant‐led care and medical management to inpatients in conjunction with the inpatient unit consultant and the MDT.
  • Contribute to multidisciplinary review and decision‐making around referrals, treatment and discharge for patients with advanced disease or terminal diagnoses, attending the weekly inpatient unit multidisciplinary team meetings.
  • Work in partnership with patients and their families/carers as a member of the multidisciplinary Specialist Palliative Care Team.
  • Provide high‐quality clinical care supported by current clinical knowledge and evidence‐based practice as part of a Specialist Palliative Care Team.
  • Provide, as needed, consultant medical care and oversight to palliative patients in their own home and care homes by working with independent nurse prescribers, clinical nurse specialists and primary‐care colleagues; undertake domiciliary visits, outpatients clinics and provide telephone support and electronic prescribing.
  • Occasionally deputise for the Virtual Ward consultants in leading daily board rounds and oversight of Virtual Ward patients.
  • Participate in on‐call rotations with Specialist Palliative Care Consultant colleagues to provide 7‐day, 24‐hour advice.
  • Adhere to and review clinical standards of care including NICE, ICN, and in‐house guidance, and contribute to the development of clinical guidelines where appropriate.
  • Help review clinical processes, policies and protocols to deliver the highest possible quality of care.
  • Support collection of accurate clinical data using PT's databases including EMIS and Sentinel.
• Education and Training
  • Support the Learning and Development team in delivering the education strategy of the hospice.
  • Provide formal and informal teaching/education in Palliative Medicine to multidisciplinary healthcare professionals across care settings, mentoring clinical nurse specialists and others studying for extended qualifications.
  • Work with the Learning and Development and consultant team to deliver education to external colleagues such as physicians, associates, paramedics, GPs, pharmacists and community nurses.
  • Act as clinical supervisor for resident doctors and specialty doctors on the Inpatient Unit, including induction, supervision and assessments.
  • Provide learning opportunities to visiting qualified medics and medical students on placement.
• Management
  • Undertake, with support, clinical service development/evaluation projects as required.
  • Maintain accurate record‐keeping and data collection, using shared records with primary and secondary care providers per guidelines.
  • Participate in regular appraisal and continuous professional development in accordance with PT's revalidation processes.
  • Assist the Medical Director by leading or supporting delegated management responsibilities across PT.
  • Engage in personal development activities such as clinical supervision, mentoring or coaching and participation in action learning sets or new consultant groups.
  • Adhere to PT's procedures, policies and guidance on operational issues, health and safety, data protection, clinical governance and confidentiality.
• Quality, Clinical Governance and Research
  • Participate in clinical governance activities including audit, medicines management, learning from deaths, incident investigations.
  • Work with others to conduct baseline assessments against best practice guidance.
  • Support the research consultant and promote a positive research culture.
  • Maintain training on Good Clinical Practice.
  • Engage in PT research activities including identifying and consenting research subjects.
• Internal Focus
  • Develop relationships with key staff, including the Medical Director, Director of Patient Services, Deputy Director of Nursing, Clinical Managers, and Ward Manager.
  • Support leadership teams in advising the Board of Trustees and Senior Leadership Team on medical issues and broader clinical issues.
  • Contribute to PT's yearly business plan and strategic plans.
• External Focus
  • Maintain up‐to‐date knowledge of external factors affecting specialist palliative care and advise on strategy.
  • Support the Income Generating team in fundraising for PT and liaise with patients and carers.

Person Specification

• Experience
  • Fully trained specialist in chosen field
  • Experience of palliation of non‐malignant disease
  • Senior‐level experience of Palliative Medicine in a variety of settings including the charitable sector

• Qualifications
  • Full registration with GMC
  • Membership with MDU/MPS or equivalent
  • Higher medical qualification
  • Eligible for Certificate of Completion of Training (CCT) in Palliative Medicine or Certificate in Eligibility for Specialist Registration (CESR)
  • Higher qualification relevant to Palliative Medicine (e.g., MSc or Diploma)
