We're looking for a registered nurse with expertise in palliative and end of life care, to join our community team, to provide specialist palliative care to patients and their families and carers in the community.

We're looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. Youll deliver the highest standards of palliative and end of life nursing care to patients, their families and carers in the communityproviding holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with Phyllis Tuckwell multidisciplinary team and colleagues across health and social care. We are excited to offer this opportunity as current post holders have been successful in progressing into more senior roles within Phyllis Tuckwell.

About You:

• A successful Registered Nurse will have:
• RGN Degree Level Qualification
• Palliative / end of life care or long-term condition experience.
• At least one year of working as a registered nurse with demonstrable competencies related to end-of-life nursing care
• Car driver and owner
• Experience of multidisciplinary team working and with external partners
• Able to make autonomous decisions demonstrating decision‑making, critical thinking and proactive caseload management

Job Purpose and Summary:

To work as a proactive member of the Phyllis Tuckwell (PT) multidisciplinary community team. The postholder will maintain the highest standards of nursing care with patients, their families and carers in the community setting. This role will involve providing holistic assessment, identification of patient needs, care planning and care provision jointly with other multidisciplinary team members. They will work alongside all community partners and with the Phyllis Tuckwell multi-professional team to ensure that the patients experience is seamless.

The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective, and efficient culture by participating in risk assessment and by working in accordance with Phyllis Tuckwells (PT) policies and procedures.

Opportunities exist to rotate from Hospice at Home onto the In-Patient Unit to further develop your knowledge and skills in palliative and end of life care and enhance awareness of the patient and family experience of end-of-life care.

Responsibilities

1.0 Clinical

1.1 To assess, plan, co-ordinate, implement and evaluate palliative care interventions based on an agreed plan of care that supports quality and timely care delivered to the patient and family or carer. Assessing the patients changing needs and liaising with other PT specialists or external partners.

1.2 To ensure that care is centred around the person, family and their carer and that effectiveness, clinical safety and a joint approach with community nursing colleagues underpins PT practice.

1.3 To work closely with PT colleagues, the District Nursing teams and other partners in the provision of personalised holistic care to patients, their families, and carers.

1.4 To ensure quality standards are maintained and constantly re-evaluated, in order to provide the highest standards of holistic care.

1.5 To ensure that clear and concise reports of clinical and other activity are maintained and monitors record keeping standards in accordance with NMC guidelines, community and PT policy.

1-6 To be able to demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace.

1.7 To act as a resource and support to palliative care colleagues through giving advice, information, support, and education.

1.8 To record clinical activity as required by the organisation and practice with due regard to confidentiality and data protection.

1.9 To report risks, accidents, incidents, negative feedback, and complaints to the Team Lead/ Manager same day.

1.10 To work with colleagues to assess their clinical competencies as appropriate, providing training and support.

2.0 Professional

2.1 To work in accordance with NMC Code and within the NMC standards.

2.2 To contribute to team cohesion, through ongoing supportive relationships and consideration of colleagues.
