We're looking for a registered nurse with expertise in palliative andend of life care, to join our community team, to provide specialist palliativecare to patients and their families and carers in the community.
We'relooking for a compassionate, proactive registered nurse to join the PhyllisTuckwell Hospice at Home Team. Youll deliver the highest standards ofpalliative and end of life nursing care to patients, their families and carersin the communityproviding holistic assessment, identifying needs, planningcare and delivering timely interventions in partnership with Phyllis Tuckwellmultidisciplinary team and colleagues across health and social care. We areexcited to offer this opportunity as current post holders have been successfulin progressing into more senior roles within Phyllis Tuckwell.

About You:
- A successful Registered Nurse will have:
- RGN Degree LevelQualification
- Current NMCregistration in adult or general nursing
- Palliative / end of life care or long-termcondition experience.
- Atleast one year of working as a registered nurse with demonstrablecompetencies related to end-of-life nursing care
- Car driver andowner
- Experience ofmultidisciplinary team working and with external partners
- Able to make autonomousdecisions demonstrating decision-making, critical thinking and proactivecaseload management

1.0 Clinical
1.1Toassess, plan, co-ordinate, implement and evaluate palliative care interventionsbased on an agreed plan of care that supports quality and timely care deliveredto the patient and family or carer. Assessing thepatients changing needs and liaising with other PT specialists or externalpartners.
1.2Toensure that care is centred around the person, family and their carer and thateffectiveness, clinical safety and a joint approach with community nursingcolleagues underpins PT practice.
1.3Towork closely with PT colleagues, the District Nursing teams and other partnersin the provision of personalised holistic care to patients, their families, andcarers.
1.4Toensure quality standards are maintained and constantly re-evaluated, in orderto provide the highest standards of holistic care.
1.5Toensure that clear and concise reports of clinical and other activity aremaintained and monitors record keeping standards in accordance with NMCguidelines, community and PT policy.
1.6Tobe able to demonstrate an understanding of issues relating to the ethicalaspects of palliative care and their application in the workplace.
1.7To act as a resource and support to palliative carecolleagues through giving advice, information, support, and education.
1.8Torecord clinical activity as required by the organisation and practice with dueregard to confidentiality and data protection.
