Phoenix Futures is looking to recruit a passionate and solution focussed Family Support Worker at HMP Pentonville. As a Family Support Worker, you will support service users and their families with their recovery journey, in HMP Pentonville and the community. You will also act as an advocate for these service users, signposting and referring to the necessary agencies to support them in their recovery holistically.
As a Family Support Worker, you will have knowledge of recovery, care planning and the ability to build and maintain relationships with internal and external agencies. You will also support the Through the Gate team in delivering psychosocial interventions to people who are within their last 12 weeks of their sentence, and for 12 weeks after release in the community. You will accompany prison leavers to their community services to assist in their engagement with commissioned services and advocate for them if needed at these appointments. All risk assessments and staffing protocols will be applied and you will not be expected to do this work in the community on your own.
