The Neighborhood at Sheffield is seeking an Activities Director (Program and Outreach Director) to join their team. The Program and Outreach Director reports directly to the Executive Director.

Purpose: The Program and Outreach Director is responsible for developing, leading and overseeing a resident-centered activities and outreach program that promotes and integrates the Phoenix Senior Living philosophy to incorporate wellness in all aspects of the seniors’ lives we serve. Ensuring all dimensions (emotional, sense of purpose, physical, social, intellectual and spiritual) are incorporated into the services we provide and embrace.

Responsibilities include but are not limited to:

  • Assisting in recruiting, hiring and training of the Assistant Program and Outreach Director.
  • Participating in the Personalized Service Plans (PSP).
  • Referring to the Resident Profile and addendum for every new resident.
  • Ensuring a variety of appropriate and personalized activities which incorporate all dimensions of wellness and are available throughout the day and evening with resident care associate participation.
  • Maintaining the PSP Activity & Program Guidebook including tracking daily which residents exhibit interest and participate in which activities.
  • Using the resident’s demographic profile information to organize small groups or clubs that address similar needs and preferences in a small group/club setting.
  • Understanding the recognition of resident changes in condition and taking appropriate action.
  • Assisting residents to and from activity room(s) when necessary.
  • In memory care, assisting residents to and from Life Skills and other normalizing life-enriching activities and routines and assisting with engagement.
  • Assisting in maintaining an inventory of activity and programming supplies, games, programs and crafts.
  • Scheduling, communicating, assisting with facilitation and recording monthly Resident Council.

Education/Experience/Licensure-Certification:

  • Education: Associate degree in Business Administration, Human Resources Administration, Gerontology or other course of study that relates to service to the elderly, preferred.
  • 1–3 years’ experience in senior living/skilled nursing activity & event planning, which may include moderate to large event planning responsibilities outside the senior living industry.
  • Must have demonstrated leadership capabilities.
  • Knowledge and experience in Assisted Living industry and Dementia care preferred.

Physical Requirements:

  • In an 8-hour workday, stand/walk: Hours at one time: 0–2; Total hours/day: 4–6.
  • In an 8-hour workday, sit: Hours at one time: 0–2; Total hours/day: 2–4.
  • In an 8-hour workday, drive: 30–60 minutes, 1–2 times a week.
  • Physical capabilities: lifting/carrying up to 50–150 lbs in various contexts.
  • Hand use for repetitive tasks: simple grasping, pushing, pulling, fine manipulation.
  • Activity limitations: Bend, squat, kneel, climb frequently; Reach occasionally (up to 3 feet).
