An exciting opportunity has arisen for one Band 6 Youth Justice Specialist Nurse post (22.5 hrs pw) working with the Youth Justice Service. You will work directly with young people to identify their health needs to reduce their risk of offending. The YJ Specialist Nursing team provide comprehensive health assessments to young people aged 10-18 who are working with the Youth Justice Service. All health appointments are delivered on an outreach basis and involve making referrals to relevant mainstream and specialist health services while liaising with NHS and council colleagues. You will have regular contact with young people to build trust and deliver an individual package of care. We are looking for an enthusiastic and motivated individual to join our friendly team. You will be a qualified SCPHN (Specialist Community Public Health Nurse) with significant experience in a role working directly with adolescents. Appointments address a wide range of health issues including physical health, mental health and emotional well-being, access to services (GP, dentist etc), sexual health, substance use, neurodevelopmental needs and safeguarding. This is a permanent post of 22.5 hours. The post is based in the East area of the county and will require attendance at clinical appointments.

Responsibilities

  • Delivering outreach health appointments with young people across mid Cornwall using youth justice CHAT assessments
  • Building trust with young people using a trauma-informed approach
  • Liaising closely with relevant professionals across health services, social care and youth justice service
  • This is a community-based role that requires travel to various locations throughout the county, including areas that may not be accessible by public transportation. Travel requirements must be managed accordingly.
