Responsibilities
The post holder will work in an adaptable and evolving community‑based role, focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings. They will implement personalised support plans, collaborate with the whole community resource, and facilitate ongoing care management. In partnership with patients, carers and the multi‑disciplinary team, they will offer support and advice, promote informed choice, maximise independence and optimise quality of life. The role requires clinical management and leadership of all staff within the team and includes frequent travel across the county to various locations, often not accessible by public transportation.
