This role is open only to current NHS employees in Cornwall or the Isles of Scilly. It is a permanent part‑time position within the North Coastal Cluster Community Nursing Team area, based in Wadebridge or Tintagel Health Offices.
Main Duties:
- Admit patients to the caseload, assess, plan, agree, implement and evaluate care.
- Work in partnership with patients and carers to offer support, advice and promote informed choice while supporting equality and valuing diversity.
- Regularly review care delivery and documentation.
- Participate in personalised care planning and promote self‑care and independence.
- Be competent and confident with core clinical skills (as per competency and training record).
- Act as a formal mentor to pre‑registration nursing students and preceptees.
- Liaise with and refer to other agencies and professionals as appropriate.
- Take full responsibility for immediate care delivery decisions.
- Offer support, supervision, information and education to patients, relatives and carers and other team members.
Availability:
Part‑time hours available on a rota covering 7 days a week from 08:00 to 22:00. Late shift hours (16:00 – 22:00) can be discussed at interview.
Person Specification – Education and Qualifications (Essential):
- Registered Nurse (Adult)
- Evidence of recent relevant personal professional development
- Current Professional Registration – NMC
- Computer literate
Desirable Criteria:
- Hold a post‑registration Diploma
- Advanced computer skills
