We are looking for a skilled and enthusiastic Band 6 Cardiac Rehabilitation Exercise Specialist / Physiotherapist to join our community cardiac service. This part-time role (0.6 WTE) offers the opportunity to deliver and develop high-quality exercise programmes for patients with cardiovascular conditions, supporting them on their journey to recovery and long-term health. You will work across a variety of settings, including community hospitals, leisure centres, clinics, and patients' homes, delivering group-based, home-based, and online exercise programmes. Working closely with the multidisciplinary community cardiac team, you will manage your own caseload, providing tailored, evidence-based care to achieve the best outcomes for patients. The role includes contributing to service development, implementing evidence-based practice, and participating in audit and research projects. You will also provide specialist guidance to other healthcare professionals, supervise and support exercise instructors and students, and help strengthen local cardiac rehabilitation networks. Main duties:
- Deliver and develop high quality cardiac rehabilitation exercise programmes for patients with a wide range of cardiovascular conditions through formal cardiac rehabilitation exercise classes, home-based, online classes and clinic-based programmes.
- Deliver and develop cardiac rehabilitation exercise programmes in collaboration with the multidisciplinary community cardiac team.
- Autonomous practitioner with responsibility for a given caseload within a defined area of the community cardiac service.
- Undertake and evaluate audit and research and implement evidence-based developments into the service.
- Provide specialist advice to other health professionals.
- Supervise, teach and appraise exercise instructors and students.
- Support and develop existing local cardiac rehabilitation networks in delivering high-quality physical activity and exercise programmes.
- Adopt a flexible and dynamic approach to patient care and service planning and development.

Person specification:
Essential criteria:
- BSc Hons Degree in Physiotherapy with experience and training in Cardiac Rehabilitation and HCPC registration, or a relevant Sports Science degree with Cardiac Rehabilitation experience.
- Membership of a relevant professional body (e.g., Chartered Society of Physiotherapy or British Association of Sport and Exercise Sciences).
- Postgraduate studies preferably MSc in Cardiac Rehabilitation/Cardiovascular Health or working towards a relevant master-level qualification.
- Experience leading and delivering Cardiac Rehabilitation exercise programmes in healthcare settings.
- Highly developed clinical skills delivering rehabilitation to patients with complex cardiac conditions, including co-morbidities.
- Experience working in a multidisciplinary team and autonomous practice.
- Presenting and teaching.
Desirable criteria:
- Level 4 BACPR cardiac rehabilitation instructor qualification.
- Lone working in the community.
