Job Description

This is a full-time position, covering shifts between 8am and 6pm, 7 days a week, including bank holidays, shared on a rota basis with other administrators. The Administrator will support the North and East Integrated Transfer of Care Hub (iToCH) in processing referrals into North and East community services. The role involves reviewing and identifying blockages in patient flow, highlighting key issues to Team Managers, and ensuring accurate and timely feedback to referrers. You will liaise with multiple agencies to support effective coordination of patient care. The successful candidate will have excellent communication and interpersonal skills, the ability to work under pressure, and a strong commitment to working collaboratively as part of our Neighbourhood team.

Duties

  • Monitor referrals on the single electronic referral system; triage in partnership with the multi-professional team.
  • Review and identify key blockages to patient flow; escalate and highlight issues to Team Managers as appropriate.
  • Maintain accurate, clear, and timely feedback to referrers.
  • Liaise with multi-agency partners to support the effective coordination of patient flow.
  • Support users with the electronic referral system.
