Job overview: We are seeking an enthusiastic and experienced AHP, particularly Physiotherapists with community experience, to join the Homefirst team in North Cornwall. This role offers opportunities to develop assessment, care planning, and reablement skills within a dynamic, multiagency team.

Main duties of the job: Responsible for patient assessment, care planning, and implementation, interpreting information relevant to patient conditions, identifying problems, and setting realistic goals. Requires excellent organisational skills and experience in older person’s services. Supportive environment with structured training, supervision, and CPD opportunities.

Person specification:

  • Essential: Degree or diploma in Occupational Therapy, Physiotherapy, or Nursing; HCPC or NMC registration; Postgraduate courses and CPD to support practice; Post-registration clinical experience in a similar role.
  • Desirable: Teaching, assessing, coaching, and mentoring skills.
  • Essential: Excellent clinical skills; Strong communication skills; Understanding of clinical effectiveness and evidence-based care.
