Cornwall Partnership NHS Foundation Trust is leading a bold transformation in Community and Palliative End of Life Care across the county. Our vision is simple yet powerful: to ensure people nearing the end of life experience a peaceful, dignified death in the place of their choosing .

Currently, Cornwall has one of the highest rates of hospital stays during the last 90 days of life, a clear signal that change is needed. We are creating a more patient-centred, community-based approach that brings compassionate care closer to home.

In collaboration with the Macmillan Social Investment Fund and local partners, we\’re expanding services including:

  • A 24/7 Rapid Community Response and Care Service
  • A dedicated End of Life Care phone line for public and professionals
  • Strengthened voluntary sector partnerships and our Hospital@Home virtual service

We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scillys.

This autonomous role involves close collaboration with commissioners, GPs, partner organisations, secondary care, community teams, and community hospitals to support the development and delivery of clinical services. The focus is on enhancing the clinical skills needed to care for patients, many with complex needs closer to home or within community settings, ensuring safe, effective, and compassionate care outside of the acute environment.

The post holder will act as the Clinical Specialist Lead, supporting patients in accordance with patient safety standards to enhance the quality of palliative, end of life care ensuring appropriate education is developed, delivered, and evaluated in relation to a wide range of clinical skills. This education will be delivered in the classroom and clinical settings working alongside clinical teams within the system to ensure appropriate access to education.

Were an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve peoples physical and mental health. We also provide specialist support to people with dementia or a learning disability.

We work in peoples homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
