We are seeking a dynamic, experienced Clinical Lead to provide expert leadership in developing our new adult community mental health Kerrier team. Position is in West Cornwall, where you'll work collaboratively to ensure high-quality care for service users. You will help shape an innovative, community-based service for individuals with complex psychosis who face barriers accessing traditional care. This is a unique opportunity to contribute directly to a new model of intensive outreach, targeting people with multiple, often unmet needs.

Your role will include applying advanced clinical expertise to assess, formulate, and guide care planning for individuals with complex presentations. You will support clinical teams in enhancing patient care pathways, especially where additional assessment is essential. Strong clinical and risk assessment skills are essential, alongside a holistic view of both health and social needs. You’ll be working in a progressive, supportive organisation that values innovation, collaboration, and staff development.

Main Duties

  • Provide specialist skills, knowledge and expertise in the assessment, care and management of all service users.
  • Assess, plan, implement and evaluate individual care plans in partnership with service users, carers, and the health and social care community.
  • Undertake comprehensive risk assessments and implement risk management strategies.
  • Demonstrate highly developed and specialised knowledge and skills in the area of Community Mental Health Services.
  • Work closely with families and carers where appropriate to reduce psychological distress associated with the caring role.
  • Provide clinical, ethical, practical and legal advice to colleagues.

We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability. Over 4,000 people make up the Trust, including doctors, nurses, therapists, and admin and support staff. We work in people’s homes, in community clinics and bases, and at 13 community hospitals.   • NMC registered & RMN professional qualification
  • Non-medical prescriber or working towards
  • Management / Leadership qualifications or current study or equivalent experience

  • Demonstrate knowledge and understanding of the principles and knowledge base underpinning crisis assessment and intervention
  • Extensive experience of conducting clinical assessments including risk assessments
  • Leadership and management skills
  • Substantial experience of working in adult mental health setting
