This role is currently open to NHS staff in Cornwall or the Isles of Scilly only. The successful candidate will hold a caseload of complex cardiac patients, liaise with primary and secondary care and support cardiac rehab exercise groups. The role involves travelling throughout East Cornwall and working autonomously and as part of a busy team.
Main Duties of the Job:
- Symptom management and optimisation of complex heart failure patients.
- Secondary prevention management.
- Education and multi-disciplinary liaisons.
- Advising primary care and regular attendance of MDTs.
- Delivering nurse-led clinics, home visits, telephone assessments.
- Participating and supporting cardiac rehab exercise groups.
Person Specification
Essential Criteria:
- Current professional registration β NMC adult.
- Significant clinical nursing experience within cardiology.
- Hold a post-registration degree or extensive relevant experience.
- Ability to prioritise and be accountable for own caseload.
- Highly developed communication skills including empathy, negotiation, motivation, reassurance and support to all patients on caseload.
- IT skills: Word, Excel, email, internet access and databases.
- Excellent clinical assessment skills, time management and organisational skills.
- Ability to work in stressful situations (including end-of-life).
- Commitment to patient safety and quality improvement.
Desirable Criteria:
- Independent nurse prescriber qualification or willingness to undertake.
- Community services knowledge.
- Heart failure experience plus relevant education.
- British Association of Cardio Pulmonary Rehabilitation qualifications.
