An exciting opportunity has arisen within the Watergate Community Nursing Team for a 30 hour Band 5 Community Nurse on a permanent contract. The applicant will need to demonstrate excellent communication skills, and sound ability to prioritise and time manage their own patient visit list within the Watergate Community Nurse Team. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation.
Main duties of the job:
- Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.
- To work in partnership with patients and carers to offer support and advice and promote informed choice.
- Responsible to support the role of the Community Nurse Team leader and other locality teams when needed.
- To work autonomously, providing effective, evidence based care in line with competency and training record and organisational policies, procedures and guidelines.

- Education / Qualifications and Experience: Registered Nurse - Adult Current Professional Registration - NMC
- To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
- Skills and Aptitude: Leadership skills, Conflict resolution skills, IT skills to include use of Word, Excel, email, internet, recording systems, Flexibility and adaptability to meet the demands of job.
Desirable:
- Advanced computer skills.
