Community Mental Health Healthcare Assistant – DOPMH Penzance

This is an exciting opportunity for an energetic and enthusiastic individual to work with people who have a dementia diagnosis or a functional mental health illness.

The Healthcare Assistant (HCA) role is part of the Dementia & Older People Mental Health (DOPMH) team.

The successful applicant will work alongside the MDT team, which comprises the Consultant Psychiatrist, CPNs, Memory Assessment Nurse, Occupational Therapist, Dementia Liaison Nurse, Psychologist and Primary Care Dementia Team.

Key responsibilities:

• Support patients and their carers in the community, either in their home or in a care placement setting.
• Deliver time‑specific care planned contact, alongside the patient’s care co‑ordinator.
• Contribute to the effective provision of Mental Health Services to an identified caseload of service users.
• Provide a range of services including practical and emotional support to individual service users as indicated by the individual care plans.

The post holder will be part of a multi‑disciplinary team, contributing to the provision of Mental Health Services to an identified caseload of service users.

The Trust is a community and mental health provider trust based in Cornwall and the Isles of Scilly, delivering community and hospital‑based care to improve people’s physical and mental health and providing specialist support to people with dementia.
