The Community Nursing Service in Cornwall is looking for highly motivated and enthusiastic Community Healthcare Assistants to join our service at Falmouth PCN. Successful candidates will demonstrate a passion for delivering nursing care to adults in their own homes, including those approaching end‑of‑life. The role offers progression via the community matrix and personal learning opportunities.

Applicants must have a valid UK driving licence (or be able to travel independently on a daily basis) and previous care experience is essential.

Key Responsibilities

• Provide personal care and end‑of‑life support to patients.
• Deliver wound care, venepuncture and other clinical tasks under the supervision of a registered nurse.
• Work as part of a community nursing team with a flexible rota covering 8 am–10 pm and an on‑call system.
• Support patients and carers in home care environments.

Person Specification

• Essential: Evidence of experience in a care setting; NVQ Level 3 qualification in Care or equivalent; GCSE Maths & English or equivalent.
• Skills & Aptitude: Ability to work autonomously within time constraints; excellent verbal and written communication; ability to cope under pressure.

Working Pattern & Contract

Permanent, full‑time position. Working pattern 8 am–10 pm with on‑call duties. Salary: £24,937–£26,598 per annum (Band 3).
