We have a very rare opportunity to join our Central Integrated Transfer of Care Hub (iToCH) as one of our administrators to support the hub with processing all the referrals coming into the Central community services.

Responsibilities:

  • Review key blockages to patient flow.
  • Highlight key issues to Team Managers and maintain accurate and timely feedback to referrers.
  • Liaise with multi‑agencies to coordinate patient flow.
  • Process and manage the flow of patient referrals through the Transfer of Care Hub and local point of access.

You must possess excellent communication and people skills; have the ability to work under pressure, whilst being a committed member of our local point of access team. This is a full‑time position covering the hours of 8 am to 4 pm, 7 days a week, including bank holidays, along with the other administrator within the team.

Main Duties of the Job: The role of the Admin Support Worker is to work with prioritizers – Nurses, Social Workers, Therapists and other members of staff employed within the multi‑professional team, processing and managing the flow of patient referrals through the Transfer of Care Hub and local point of access. The Admin Support Worker will monitor the referrals on the single electronic referral system on a continual basis, triaging in partnership with the multi‑professional team.

Person Specification – Qualifications:

  • Essential Criteria: NVQ 3 Administration or equivalent.
  • Experience of working within a community setting.
  • Good standard of education including GCSE English & Maths or equivalent.

Experience, Skills and Aptitude – Essential Criteria:

  • Ability to prioritise own workload and work unsupervised with accuracy and attention to detail.
  • Previous experience in a healthcare office environment.
  • Computer literate with an excellent working knowledge of Microsoft Office, Excel and PowerPoint.
