We are looking for a Private Duty LPN in Highland, IL.
Pay Rate: $28.00/hr-$33.00/hr

Responsibilities:
- Assist in developing and revising the care plan.
- Provide nursing care per the physician’s treatment plan and client’s payer authorization.
- Monitor, record, and report treatment reactions and changes in condition to relevant personnel.
- Provide ongoing instruction to clients, families, and healthcare team members.
- Record patient condition and care services in electronic charts.
- Participate in educational programs, quality assessments, and staff meetings.
