About the Role
Phix Health is looking for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join us at an outpatient practice in and around Philadelphia, PA. This is a part-time opportunity for 2-4 days per week, depending on location.
Responsibilities:
- Provide compassionate primary care services for geriatric and/or disabled patients including comprehensive and focused physical examinations, health assessments, diagnoses, and treatment of acute and chronic illnesses.
- Manage chronic conditions such as diabetes, hypertension, congestive heart failure, arthritis, chronic obstructive pulmonary disease, and thyroid disorders.
- Assess need for appropriate lab tests, order and interpret results.
- Collaborate with medical specialties through appropriate referrals.
- Assist in coordination of community resources to meet needs of individuals and families, as well as appropriate follow-up.
- Educate patients on health promotion throughout the aging process.
- Evaluate and treat patients in accordance with Phix Health’s Care Model.
- Establish and monitor appropriate level of care for center patients.
- Complete all medical record documentation prior to end of shift.
- Work with Medical Director to ensure care operations are consistent with medical and professional standards of care.
- Recommend potential growth opportunities for new or existing services.
- Meet with Medical Director or designate to discuss quality of patient care, review policies, procedures, outcomes reports, and records.
- Participate in quality management processes including encouragement of clinical support staff to assure proficiency in the delivery of clinical support activities.
- Call patients/family members to ensure their well-being as needed following center visits.
- In the absence of Medical Director, provide guidance to other medical personnel and center colleagues.
- Perform all other related duties as assigned.
Qualifications:
- Assure personal compliance with licensing, certification and accrediting bodies.
- Licensure requirements of the state of jurisdiction.
- Graduation from an accredited Nurse Practitioner or Physician Assistant program.
- Procedural skills are a plus.
- Prior primary care experience and experience as a visiting nurse a plus.
- Comfortable using EMRs, MS-Teams and technology in general a must.
